Dilemas da saúde: financiamento e descentralização

Abstract

Aperfeiçoar o financiamento e a descentralização das ações de saúde da União exige visão e soluções integradas com as reformas tributária e orçamentária. Após a Constituição de 1988, aumentou, e muito, a presença dos Estados e Municípios, e o orçamento federal tornou-se marcantemente dependente das contribuições para a seguridade social. Os programas federais de saúde pouco se beneficiaram porque foram comprimidos pela expansão da previdência e ficaram mais expostos à política de contenção do Tesouro Nacional, não tendo sido montado um adequado esquema de inter-relacionamento com Estados e Municípios. O equacionamento financeiro da seguridade – especialmente o da saúde – nem tão cedo poderá prescindir das atuais contribuições sociais. A vinculação constitucional de receitas não é uma alternativa racional e segura. Propõe-se buscar mudanças graduais e criativas e privilegiar os esforços para melhorar a gestão de saúde pública.

The improvement of funding and decentralization of the Government's health actions require wise and integrated solutions involving tax and budget reform. After the 1988 Constitution, the presence of states and municipalities has greatly increased, and the federal budget has become highly dependent on social security contributions. Federal health programs could not benefit much because, first, they have been compressed by an increase in security payouts; second, they became more exposed to the reduction policy by the National Treasury; and, third, this process has not been followed by a suitable arrangement between federal, state and municipal levels. The equation of social security – as far as health is concerned - cannot be achieved in the short term without the current contributions. Constitutional proposed is the search of gradual and creative changes and an effort to improve public health management.
